Aimed at patients recently diagnosed with type 2 diabetes, our courses are tailored to improve self-management of the disease, lower HbA1c, and achieve sustainable behaviour change. By supporting patients in setting their own goals and personal action plans, we aim to maximise patient benefit and understanding.

Clinical and administrative teams refer patients to Second Nature with a simple form

Patients are sent a sign up link via email and text, with 75% signing up within 24 hours

12 weeks of support from a health coach and peers to improve self management

Patients retain access to all programme content for as long as they need it

Inclusion Criteria

  • Aged 18+
  • Living with type 2 diabetes
  • BMI 30+ (27 + for Black, Asian, and Minority Ethnic).


Exclusion Criteria

  • Not pregnant or planning pregnancy
  • BMI less than 18.9
  • Not diagnosed with type 2 diabetes
  • The patient must not have a diagnosed active eating disorder
  • The patient must not have had bariatric surgery in the last 2 years even if the surgery has been reversed. 
  • The patient must not be identified as having moderate or severe frailty.
